About the service

Prime Link Care Pty Ltd provides Supported Independent Living (SIL) to help NDIS participants live safely, confidently, and as independently as possible in their own homes or shared living environments.

Our SIL services are designed to support participants with daily living tasks, personal care, household activities, and skill development, while promoting choice, control, dignity, and independence.

What is Supported Independent Living (SIL)?

Supported Independent Living is an NDIS-funded support that assists participants who require ongoing support to live in a shared or individual living arrangement. SIL focuses on building independence, improving daily living skills, and supporting participants to engage in their community and achieve their personal goals.

Our SIL Support Includes:

Daily Living Support:

  • Personal care (showering, dressing, grooming, toileting)
  • Medication reminders and health monitoring
  • Support with meals and nutrition
  • Assistance with mobility and transfers

Household and Domestic Support:

  • Cleaning and laundry
  • Meal preparation and cooking support
  • Household organisation and budgeting assistance
  • Maintaining a safe and comfortable living environment

Skill Development and Independence Building

  • Developing daily living skills
  • Promoting self-care and personal responsibility
  • Supporting decision-making and problem-solving
  • Building routines and life skills

Community and Social Participation

  • Support to attend appointments and activities
  • Community access and social engagement
  • Support to maintain relationships with family and friends

Person-Centred and Participant-Led Support

At Prime Link Care Pty Ltd, we take a person-centred approach, ensuring that every participant is actively involved in decisions about their supports. We work closely with participants, families, guardians, and support coordinators to develop individualised support plans that reflect each participant’s goals, preferences, cultural background, and needs.
